Transportation for the appointments that matter, again and again.

Sunny Breeze Transport provides ambulatory non-emergency medical transportation (NEMT). Our current service is for passengers who are able to walk and do not require wheelchair transportation.

Recurring transportation is a core part of what we do. Whether you are arranging your own rides or helping a loved one, call us to discuss regular pickup locations, destinations, appointment days, and return trips. Each schedule is reviewed and confirmed with our team or the applicable broker.

CHILDREN & FAMILIES

ABA & pediatric care

We currently serve ABA clinics for children with autism. We can also discuss transportation to Prescribed Pediatric Extended Care (PPEC) centers and pediatric appointments for children whose travel needs fit our ambulatory service. Families and care teams should confirm any required support before arranging a trip.

Adult day care

We serve adults traveling to dialysis and can discuss recurring transportation for other outpatient treatments. Share your regular appointment schedule so we can review arrangements, availability, and any broker coordination needed. Ambulatory service only.

TESTS & FOLLOW-UPS

Labs, imaging & follow-up care

Discuss transportation to laboratory tests, diagnostic imaging, and follow-up appointments. We provide transportation, not medical care during the ride.

Broker-coordinated transportation

We work through transportation brokers that coordinate rides for health plans, including Medicaid and Medicare-related plan programs.

Contact your health plan or its transportation broker to confirm your transportation benefit, eligibility, authorization, and booking process. Coverage and provider assignment depend on your specific plan.

  2. Confirm authorization and provider assignment.Your plan or broker determines eligibility and coordinates the applicable trip arrangements. A provider preference does not guarantee assignment; approval depends on the applicable program, network participation, and availability.
  3. Check your ride details.Follow the instructions from your coordinator. If Sunny Breeze is assigned, contact us with questions about the trip.

Can I ask my broker for Sunny Breeze Transport?

When scheduling your ride, ask whether your broker can record Sunny Breeze Transport as your preferred provider. The broker or health plan determines whether that request can be accommodated. Assignment is subject to program requirements, network participation, and availability.

Can a clinic arrange transportation?

Clinics, facilities, and brokers can call us to discuss ambulatory transportation needs. For rides covered by a health plan, booking and authorization must follow that plan or broker’s process.

Can we discuss recurring appointments?

Yes. Call with the recurring days, pickup and destination, and appointment times so we can discuss availability. For broker-arranged rides, confirm recurring-trip arrangements with the plan or broker.

Can I request a return trip?

Tell us whether you need a one-way or return trip when discussing a private-pay quote. For broker-arranged rides, confirm both legs with your plan or transportation coordinator.

How far ahead should I call?

Contact us as soon as you know your appointment details to discuss availability. For broker-arranged trips, ask your plan or broker about its advance-booking requirements.

Who is this service for?

Our current service is ambulatory transportation for passengers who can walk. Tell us about your mobility needs when you call so we can discuss whether our service is suitable.

Do you provide wheelchair transportation?

Wheelchair transportation is not part of our current service. Sunny Breeze Transport currently provides ambulatory transportation only.
